Preferência do paciente no rastreamento do cancer colorretal: uma compara??o entre colonografia por tomografia computadorizada e colonoscopia

Abstract: objective: to assess the degree of acceptance of patients undergoing computed tomography colonography (ctc) in comparison with colonoscopy in the screening of colorectal disease. materials and methods: fifty patients with suspected colorectal disease underwent ctc and colonoscopy. questionnaires were administered before and after the performance of the ctc and after the colonoscopy. the discomfort expected and experienced before and after the performance of both procedures as well as the patients' preference for each method were evaluated. results: as regards ctc, before the procedure, 18% of the patients reported expecting little discomfort, 78%, mild discomfort, and 4%, a lot of discomfort. after the procedure, 72% of the patients reported little discomfort, 26%, mild discomfort, and only one (2%) of the patients reported a lot of discomfort. upon completion of the colonoscopy, 86% of the patients reported their preference for ctc. the degree of colonic distention and residual amount of fluid had no influence on the patients' preference. conclusion: ctc was preferred to colonoscopy, with no statistical relationship with the degree of colonic distention at ctc and efficiency of bowel preparation.

Abstract: computed tomography colonography is a minimally invasive method for screening for polyps and colorectal cancer, with extremely unusual complications, increasingly used in the clinical practice. in the last decade, developments in bowel preparation, imaging, and in the training of investigators have determined a significant increase in the method sensitivity. images interpretation is accomplished through a combined analysis of two-dimensional source images and several types of three-dimensional renderings, with sensitivity around 96% in the detection of lesions with dimensions equal or greater than 10 mm in size, when analyzed by experienced radiologists. the present pictorial essay includes examples of diseases and pseudolesions most frequently observed in this type of imaging study. the authors present examples of flat and polypoid lesions, benign and malignant lesions, diverticular disease of the colon, among other conditions, as well as pseudolesions, including those related to inappropriate bowel preparation and misinterpretation.
Comparative histological study of atherosclerotic lesions and microvascular changes in amputated lower limbs of diabetic and non-diabetic patients

Abstract: objetive: to perform a comparative analysis of atherosclerotic lesions and capillaries changes in diabetic and nondiabetic patients. methods: leg arteries and skin of 57 amputated lower limbs of diabetic (47.3%) and nondiabetic patients were histologically examined. the percentage of arterial stenosis of infrapopliteal arteries and the histological classification of atherosclerotic lesions were determined. capillary thickening was classified into four categories. results: diabetic group showed more than 75% stenosis in 57% (vs. 56% in nondiabetic) of the anterior tibial; 78% (vs. 68%) of the posterior tibial; 58% (vs. 50%) of the peroneal leg arteries. diabetic and nondiabetic patients have predominance of type vi atherosclerotic lesions. the comparison of both groups showed no significant differences in atherosclerotic lesions. diabetic patients had significantly more pas positive capillary thickening (63% vs. 23%). conclusions: there were no differences in histological characteristics of atherosclerosis between the two groups. capillary thickening has been more observed in diabetics.
Analysis of the effects of the interruption of the flow in the normal arterial endothelium morphology and the correlation with the ischemia duration in rabbits

Abstract: the study of acute arterial insufficiency of the extremities is an area of continuing interest and investigation, in light of the possible effects stemming from the evolution of the disease and the necessity for urgent treatment. purpose: to analyze the effects of the interruption of the flow in the normal arterial endothelium morphology and correlate them with the ischemia duration. methods: we submitted 30 rabbits to the ligature of the right external iliac artery for 6 hours or 72 hours and observed specific morphological variables in the endothelial layer under optical and electronic microscopy. results: in the optical microscopic study, no statistically significant results were observed in the comparison of the groups (control, 6- and 72-hour occlusions). with electronic microscopy, we observed alterations in the endothelial cell characterized by hyperpigmentation with detachment of the same from its bed; and rupture of the internal elastic membrane, with the exposure of the subendothelial material to the vascular lumen. conclusions: the optical microscopy was not an effective method for the determination of endothelial morphological alterations; the electronic microscopy allowed us to observe initial signals of the endothelial cell and layer injury 72 hours after the interruption of the normal arterial flow.

Abstract: we report an unusual case of association of plastic bronchitis (pb) to protein-losing enteropathy (ple) in a girl of 4 years and 9 months of age with double inlet single left ventricle and ventriculoarterial concordance. submitted to total cavopulmonary surgery. with an intracardiac lateral tunnel at the age of three. the elimination of the 10 cm fibrin bronchial mold (pb) and the alpha-1-antitrypsin elevation of 52 mg/g in feces had both become outstanding. using sildenafil. the thoracic duct ligature and the cardiac transplant were programmed in case of continuity of the process.
Mortality within the endovascular treatment in Stanford type B aortic dissections

Abstract: background: endovascular stent-graft repair of aortic dissections is a relatively new procedure, and although apparently less invasive, the efficacy and safety of this technique have not been fully established. objective: to evaluate mortality in patients with complicated stanford type b aortic dissections submitted to endovascular treatment. methods: clinical, anatomical, imaging and autopsy data of 23 patients with complicated type b aortic dissections were reviewed from november 2004 to october 2007. the main indications for transluminal thoracic stent-grafting included: persistent pain in spite of medical therapy, signs of distal limb ischemia, signs of aortic rupture, progression of aneurismal dilation of the descending aorta during follow-up (defined as a diameter > 50 mm) and the diameter of descending thoracic aorta of 40mm or larger at the onset of aortic dissection. data were analyzed statistically; all p-values were two-tailed and differences < 0.05 were considered to indicate statistical significance. continuous variables were expressed as mean (± sd), and medians were compared by the student's t test. differences in categorical variables between the groups were analyzed by the chi-square or fisher's exact test. results: the procedure presented primary technical success in 82.6% of patients. four patients (17.4%) had an incomplete proximal entry seal. three patients (13%) died within 30 days of the procedure and eight patients (34.8%) died after 30 days. conclusion: endovascular correction of complicated stanford type b aortic dissections is a feasible and effective treatment option.

Abstract: First launched in 1982, JECCR joined BioMed Central in order to provide rapid dissemination of scientific results to the widest possible global audience. Furthermore, authors submitting to JECCR can expect a fast turnaround time, benefitting from an efficient review process and publication immediately upon acceptance. JECCR's content will be widely indexed (for instance by PubMed, Medline, Thomson/ISI, Embase, and CAS) and covered by a range of freely accessible full-text archives [3].JECCR publishes scientific studies on the biological, epidemiological, immunological, pathological, radiobiological, and clinical aspects of oncology. Topics considered range from molecular genetics via infectious agents to surgery and therapeutic approaches and outcomes. The broad oncological focus, combined with the Editors' experience in evaluating and conducting systematic reviews and developing clinical practice guidelines, makes JECCR an attractive forum for publishing reviews and guidelines. This focus, well documented in an Editorial by H. J. Schünemann [4], goes very well together with the approach now being widely applied by many international and national organizations, including the World Health Organization (WHO), Cochrane Collaboration, American College of Physicians (ACP), and the National Institutes for Health and Clinical Excellence in the U.K. (NICE).In response to increasing requests JECCR has decided to devote particular attention to more specific areas of cancer investigation. To this end, specific sections and subsections under the supervision of expert Editors have been created: the new topics include Gastrointestinal Stromal Tumors (GIST), Lung and Pleural Tumors, and Preclinical Investigations in spontaneous animal models.We believe that the new editorial changes combined with the advantages of BioMed Central's open access publishing platform will greatly improve JECCR's dissemination and visibility. Abstract: in the present paper i consider an argument of jaegwon kim's, presented in the third chapter of his physicalism or something near enough, which purports to show the inadequacy of substance dualism as a solution to the problem about the nature of mind. i intend to show that cartesian dualism is beyond the reach of kim's attack, provided that all the implications of descartes use of the theory of natural institution and of the notion of a psychophysical union are taken into account. moreover, kim's arguments directed towards the question of mind's nonspatiality, and its derived impossibility to enter into causal relations, seem to vanish, once we take into account, on the one hand, the fact that the cartesian man involves a union of mind and body which is such that they may be regarded as co-extensive and, on the other hand, the particular way, very near to occassionalism, in which descartes explains interaction.

Abstract: The diffusion of Internet is changing the role of archives and libraries and it is opening a wide range of new possibilities. The new vision is that in few years it will be possible to cross-access multiple libraries, archives, museums, and data repositories. The implementation of this revolutionary vision requires the solution of a number of technical, organizational, sociological, and economical issues. Recently, the proposal of a new, low cost technical solution for open repositories of pre-print material, the Open Archives Initiative Protocol for Metadata Harvesting ( OAI-PMH), has greatly stimulated the discussion about these issues in many application frameworks. This paper overviews the status of this discussion in the library and conventional archives frameworks.
